Passive smoking — also called second-hand smoke (SHS) exposure — is the involuntary inhalation of smoke from someone else's cigarette, cigar, or pipe. It contains over 7,000 identified chemicals, of which at least 70 are known or probable carcinogens. Passive smoking causes approximately 12,000 deaths in England annually (ASH UK) and is the single most potent non-infectious asthma trigger for many UK patients.
Irritant Rhinitis vs IgE-Mediated Tobacco Allergy
The most common UK presentation is irritant-induced rhinitis — where smoke chemicals directly damage the nasal mucosa without IgE involvement. This will show negative allergy tests. A smaller proportion develop genuine IgE-mediated tobacco allergy confirmed by skin prick testing. Both produce identical symptoms: sneezing, runny nose, watery eyes, throat irritation, coughing, and chest tightness.
Protecting Yourself from Passive Smoke
Medical management: non-drowsy antihistamines (cetirizine or loratadine), corticosteroid nasal sprays (mometasone or fluticasone), and up-to-date asthma inhalers if required.
